We are happy that you decided with your association or school for an entry into our Budo umbrella organisation. In this form you have now the possibility to announce officially and to register yourself and your club/school in the A.D.D.I. These from you to us transmitted data are automatically processed by our administrative system and are the basis for the issuing of the membership cards and other membership documents. In the following form please complete all necessary data of your club/school. Please fill out carefully and completely all necessary fields, so that a smooth and fast processing of your application is guaranteed. Please note that with a first registration for clubs/schools a member size of minimum 6 students per club/school exists and is to register in the A.D.D.I. as well. Please wait with the student registration form until you have received from our administration your official letter of acceptance with your club/school membership number. Data protection is taken seriously with us! We obligate ourselves to always treat all and informed data confidentially and to only use them to the extent necessary for the processing of the applications, as well as not to transmit these to third parties. PART III - APPLICATION EXAMINER LICENCE -
Herewith I / we apply in the name and on behalf of my / our school / association / club a style-related and teaching-related association-examiner's licence for the certification of examinations in the name and on behalf of our school / association / club and the A.D.D.I. Martial Arts organisation.

APPLICATION FOR ISSUING EXAMINER LICENCE Herewith I/we apply in the name of or/my school/club a style- and teaching-related association-examiner's licence for the certification of student examin in the name and on behalf of our school or club and the A.D.D.I. Martial Arts Organization. I/we After my/our application submission on the issuing of an official association-examiner's licence, I/we receive the applied examiner's licence of the chosen examiner's licence type (A / B / C). This licence package includes; registration in the sports pass (licence), personal examiner-licence certificate, examiner-licence identity card and examiner-licence stamp. The issued examiner's licence is not transferable to third parties and is valid only for all examinations within the scope of a school, association, or club activity and only to its members. The examiner's licence is valid for a period of 2 calendar years provided that this does not become invalid on account of an untimely leave from the association or a coarse offence against the statutes and orders of the A.D.D.I. In this case the licensee is informed automatically immediately. I/we take note furthermore that my / our activity as a „licensed association examiner“ is valid only with the corresponding and from the association provided means. This also includes an original association examiner's stamp with registration number, an examiner's licence identity card, as well as the exclusive use of the original and official examination certificates of the A.D.D.I. which are to be received only through the office (order of materials). Own certificate productions or duplications of original organisation certificates of the A.D.D.I. are not admitted without the written approval of the office and are therefore invalid. Deviating arrangements are only valid with a written confirmation of the presidium. A violation of this regulation and/or of a determined article in the association regulations or statutes can mean the loss of my examiner's licence as well as an exclusion from the organisation for lifetime for the licensee and also for the school/club. With the submission of this application for an issuing of an examiner's licence I/we regard obligingly and without restrictions the organisation statute and examination statute as well as the terms and conditions of the A.D.D.I. MARTIAL ARTS ORGANIZATION.

APPLICATION SUBMISSION EXPLANATION Herewith I am/we apply as a main applicant of my/our club/school / the membership as main member in the A.D.D.I. and declare that the provided information in this application is correct and complete. I am/we regard without restrictions within the scope of the application and membership the published statutes & regulations of the A.D.D.I. Martial Arts Organisation as an obliging component of our membership. Furthermore I am/we apply for the concurrent issuing of a chargeable examiner's licence A / B / C for my/our club/school. This so called examiner's package contains: 1 examiner's licence identity card, 1 examiner's stamp and 1 examiner's licence certificate issued according to the above information to our/my club/school. At the same time I ask for the creation and activation of a shopping account in the AMOOS system for my/our club/school.
